JONATHAN SANTOS-RAMOS NAMED CFH EXECUTIVE DIRECTOR

Care For the Homeless is proud to announce that Jonathan Santos-Ramos has been appointed Executive Director. Previously CFH’s Chief Program Officer, Jonathan steps into this role with deep experience in healthcare administration, a strong track record of mission-driven leadership, and a clear commitment to advancing CFH’s integrated model of healthcare and shelter for New Yorkers experiencing homelessness.
Jonathan joined CFH in June 2023 and quickly became a trusted leader across the organization—bringing both strategic focus and a people-first approach to some of our most complex work. His appointment recognizes not only his professional experience, but also the perspective he brings to CFH’s mission: a lifelong commitment to community-based care, and a belief that dignity, health, and stability must be accessible to every person, regardless of housing status.
“I’m excited to take a larger role in an organization that I believe so much in,” Jonathan said. “More now than ever, I feel we have a real opportunity to make an impact on homeless healthcare and shelter in New York City. The way that we do it is pretty unique, and to be able to lead staff who do this amazing work is really exciting.”
That unique approach has always been at the heart of CFH. We know that homelessness and health are inseparable—and that meaningful progress requires more than one service at a time. CFH brings care directly to the places where it’s needed most, delivering healthcare alongside shelter and supportive services, and helping people move from crisis toward stability. Jonathan has been a champion of that integrated model, and he has played a key role in strengthening the systems, teams, and partnerships that make it possible.
Jonathan’s pathway into healthcare leadership began early, rooted in service and shaped by real community experience. As a teenager, he started as a peer educator in a nonprofit setting—his first summer job at age 14—supporting health education programming and learning firsthand how prevention, trust, and clear information can change lives. That early experience became the beginning of more than two decades of work in healthcare administration and community-based programs.
Raised between Puerto Rico and New Jersey, Jonathan brings a personal understanding of what it means to navigate different communities, identities, and systems—and to stay grounded in what people need to feel supported and seen. He moved to New Jersey in 1997 to pursue his undergraduate degree at Rutgers University, and later earned a Master of Public Administration (MPA) from Baruch College. Over the course of his career, he has built extensive experience across healthcare administration and nonprofit leadership, with a consistent focus on improving outcomes for people who have historically been underserved or overlooked.
In addition to his work at CFH, Jonathan has long contributed to the broader social-impact ecosystem through board and advisory involvement, reflecting his dedication to community, equity, and civic leadership. His service has included involvement with organizations such as the Brooklyn Community Pride Center and the National Health Care for the Homeless Council, among others.
As Executive Director, Jonathan will continue working closely with CFH leadership, staff, and partners to advance our mission and strengthen our capacity across programs. We are excited for Jonathan as he takes on this new role, and we are grateful for the leadership he has already provided to CFH.
